I follow the book that came with the car, supplied by Isuzu. But interesting the driver's manual for the last of the Holden Colarado' s recommended using Dexron VI in the manual gearbox. For what its worth, I'm currently in the country house where I keep the Dmax. I had a look in the Haynes manual for their recommended manual transmission oil and it's Dexron VI.

Can you read any fault codes? A sticking or faulty SCV will log rail pressure fault codes.

If you have the 4JJ1 engine you have glow plugs. I'd have a look at them and the starting sequence timing.

Check with Amsoil in Costa Rica. Amsoil Manual Transmission Fluid 5W-30 is supposed to be equivalent to the Besco oil.

Have a look at your manual, mine says use Besco (5W-30) transaxle oil. I know Besco is hard to find in Australia but I'd use an equivalent transaxle oil, especially if a noise developed after switching to synthetic engine oil like's happened to you. I used to get low EGR gas flow codes when I was pulling a boat up a ramp and had an EGR restrictor plate plate fitted but they were easily cancelled by OBD2 Autodoctor and didn't recur until the next time I retrieved the boat. This led me to remove the plate and fit a catch can.
