Try getting someone under the bonnet whilst doing a cold start-up and rev the engine manually when throttle pedal doesn't work.
That would discount the DBW.
If the fuel pump in the tank is cheap replace it.
Dark smoke is unburnt fuel, you could have a tight valve = low compression on a cylinder.
I had a truck with a tight valve, it missed on cold start-up but went away when hot.

How many Kms on the car?

A diesel specialist may be more help than a holden dealer.

I bought a 2007 rodeo a couple of months back and I'm having the exact same issues, had anyone found out what the problem actually is and how much it costs to get fixed?

My 2012 current shape 3L auto LSM Dmax 62000km
Stopped at traffic lights and noticed rough idle, traffic lights turn green and I try to accelerate away but it wont rev just sort of idles then after 5 to 10 secs it accelerates away and maintains traffic speed till the next stop and repeats again.

Mine was the EGR valve.

Pretty disappointed that the valve seized in the open position after only doing 62000km.
Isuzu fitted a new one and presently I'm pulling the old one to pieces for shits and giggles. I think I can recondition it and keep as a spare.

Should've just put a restriction plate in when it was out and been done with it.

In my personal ute, I have by-passed this mess altogether.
On another note we have 3 others of similar age (ca. 2012 new shape) that are now well over 200k each now and still going. No EGR issues.
My theory, I think alot of extended driving helps EGR systems, short driving not so much.
..all Dmax's.

Initially I had no idea what the problem was as no indicator lights were thrown up on the dash. I had just filled up at a servo and thought it was the fuel.
Isuzu dealership diagnosed the problem and fitted a new one, buckleys chance of them fitting a blanking plate at the time for obvious reasons .
I am in the process of getting a slotted blanking plate but am also going to look into whether my inlet manifold needs stripping and cleaning. I've heard of the threebond product but not sure if I should go down this path.
Driving to and from work 30mins with a constant 90kph stretch for 15mins each way, I would have thought would have been ok. I'm thinking maybe driving a bit more aggressively as in acceleration in city driving might be better for non clogging of EGR.

Did anyone work out what the problem was ( Re: Rough idle with no acceleration ), my 3L diesel has just started doing the same thing.
